Sophie, a bright and active girl, lives at home with her family. Despite her severe respiratory disease at birth, Sophie now thrives with the support of a dedicated care team. Due to her condition, she has a tracheostomy and requires 1:1 nurse assistance to attend school. Innovate Care Group has been instrumental in developing a comprehensive care plan to address Sophie’s medical and emotional needs.

Jemima journey with Innovate began with a thorough assessment and engagement. Jemima, who had a spinal cord injury resulting in tetraplegia, was keen to regain a sense of normalcy and independence despite her significant physical challenges. Innovate’s team worked closely with Jemima and her family to develop a care plan that addressed her complex medical needs while also focusing on her emotional and social well-being.

Lucy has a diagnosis of Cerebral Palsy, which presents a range of complex health needs, including respiratory health, mobility challenges, digestive health issues, epilepsy management, and multiple learning difficulties. Despite these challenges, Lucy’s journey with Innovate Care Group has been one of empowerment and active engagement in life.
